Becoming A Joslin Patient 

Your diabetes care begins with your personal physician.  The educators at the Joslin Diabetes Center Education Affiliate at St. Mary's work closely with you and your physician.  Referring physicians receive a comprehensive report so they can continue to attend to your overall healthcare needs.

To participate, you must be referred by a physician.  When Joslin receives the referral for self-management education or training, you will be contacted by one of our staff members to set up an appointment.  It is recommended that you contact your insurance company before coming to us for your appointment to be sure this educational service is covered.  Medicare does cover diabetes education for appropriate patients.  Payment plans and other financial options are available.

Your initial visit to Joslin will consist of:

  • Personal history
  • Introduction to our program
  • Individual visit with a registered nurse and registered dietitian

This one-hour visit includes instructions on home blood glucose monitoring, desired ranges for blood sugars and introduction to meal planning. You will then be scheduled for your first class.
