What services are provided?
St. Mary’s At Home provides comprehensive, multidisciplinary in-home health care services to promote, maintain and restore health. Our services are delivered in accordance with a physician's orders and as defined in a plan of care. St. Mary's At Home has been approved by The Joint Commission and is state accredited.
Our homecare professionals will work with you and your physician to provide the medical attention you need in the comfort of your own home. The team is comprised of:
- Nurses
- Physical Therapists
- Occupational Therapists
- Speech Therapists
- Home Health Aides
- Social Workers
Home care services are provided to:

Ease the transition from hospital or health care facility to home

Return patients to maximum health

Provide counseling and coordination of community services to meet patients’ needs

Maintain patients’ level of functioning, within their capabilities, lifestyle and stage of life

Administer quality health care services within the home

Instruct patients and caregivers in the fundamentals of health, hygiene and safety
Nursing Services Nursing services are provided by Registered Nurses (RN's) and Licensed Practical Nurses (LPN's). Examples of skilled nursing include:
- Nursing assessment and education regarding disease management
- One-on-one education concerning medication
- Pain management
- Wound, ostomy, and catheter care
- Diabetic care management and teaching
- Promote self care with the highest level of independence
- Assistance with equipment management
- IV therapy
Home Infusion Division The home infusion division provides patients with IV therapy in the home. The infusion team consists of experienced registered nurses and pharmacists who work collaboratively with physicians, dieticians, and caregivers to offer a positive alternative to hospitalization.
Physical Therapy Services Physical Therapists specialize in treatments designed to improve the mobility and physical strength of patients. Physical therapy consists primarily of physical exercise. Therapies are used to help alleviate pain and restore muscle strength. Physical therapy services offered:
- Gait and transfer training
- Therapeutic exercise
- Ultrasound and electrical muscle stimulation
- Neuro-muscular re-education
Occupational Therapy Services Occupational Therapists specialize in treatments that enable patients to perform daily living activities such as bathing, eating, dressing and household routines. Individuals with physical, developmental and/or social problems might need the skills of and occupational therapist. Services provided by occupational therapists may include:
- Perceptual motor training
- Splinting
- Equipment adaptation
- Self-care activity training
- Muscle training
- Energy conservation
Speech and Language Therapy Services Speech and language pathologists specialize in treatments to restore speech that may have been lost due to some form of trauma. Speech therapists also train patients to improve breathing and swallowing. Speech and language therapists offer the following services:
- Alternate communication methods
- Voice and speech retraining
- Language restoration
- Esophageal speech training
- Cognitive retraining
- Oral/facial exercises
- Swallowing disorder/treatment
Medical Social Services Medical Social Workers evaluate social and emotional factors that affect the ill and disabled. They help patients and families manage the stress that many experience, and they connect them with available community resources. Medical social workers provide:
- Counseling
- Community resource planning and referral
Home Health Aids Home Health Aids assist patients in the completion of daily living activities and personal care.
Home Safety Evaluations A Registered Nurse or occupational or physical therapist will evaluate the home environment for safety risks. Individualized assessments reflect the client's home environment and situation. The nurse or therapist instructs the client on areas of improvement that could assist in reducing safety risks.
